What is Pathways to Community Living?

Pathways to Community Living (Money Follows the Person-Tribal Initiative) is a grant focused on keeping elders in their homes by establishing sustainable and culturally appropriate long-term services and supports in tribal communities.

It transitions interested and qualified Oklahomans from a nursing home or rehabilitation center back into their communities.


Program Benefits

  • Provides additional support beyond regular SoonerCare benefits
  • No additional contracts
  • Ensures tribal sovereignty
  • Partnership and support with OHCA’s Tribal Government Relations Unit

Help for Tribes and Tribal Members

  • Provide medical equipment to members
  • Modify homes to make them more accessible and safer
  • Hire staff to enhance services or offer new services
  • Reimbursement for services your tribe already offers, such as delivered meals, mental health services, home visits, etc.
  • Funds for leasing vehicles that can provide transportation to and from more than just medical appointments

Covered Services

Final services will be determined in consultation with the participating tribes but may include:

  • Personal care
  • Skilled nursing
  • Case management
  • Adult day services
  • Transportation
  • Delivered meals
  • Limited dental services
  • Mental health counseling
  • Self-direction for services
  • Transition funds for housing needs

2023 Impact

  • $3.2 million given to Pathways-partnered tribes in Oklahoma
  • Over 20 meetings with 4 other participating states (Minnesota, North Dakota, Washington, Wisconsin)
  • Co-presented on a national platform in Washington, D.C.
  • Nearly 70 meetings with participating or interested tribes
  • 6 current grant partners
