Required Documents


Before You Begin

Depending on the service that you are looking to complete, you will be required to provide acceptable documents. Currently, there are three categories of documents: Proof of Identity, Proof of Oklahoma Residency, and Proof of Lawful Presence. Acceptable documents for each are listed below.

Proof of Identity Documents

You will need to provide Proof of Identity. The document must be a certified original or a certified copy. Notarized documents are not accepted.

Examples of Proof of Identity include:

  • A valid Oklahoma Driver License or State ID Card not expired more than three years or a valid, non-expired out of state credential may satisfy this requirement. For CDL, the driver license or state identification card cannot be expired
  • Birth certificate
  • Consular Report of Birth Abroad issued by the U.S. Department of State (Form FS-240, DS-1350, or FS-545)
  • Valid, unexpired U.S. Passport or Passport card
  • Valid, unexpired Permanent Resident Card issued by U.S. Department of Homeland Security or USCIS (Form I-551)
  • Certificate of Naturalization or Citizenship issued by U.S. Department of Homeland Security (Form N-550, Form N-570, N-560, or N56); Approval certificates will not be accepted
  • Valid, unexpired employment authorization document (EAD) issued by U.S. Department of Homeland Security (Form I-766)
  • Valid, unexpired foreign passport with a valid U.S. Visa and valid I-94 issued by U.S. Department of Homeland Security. Other USCIS documents may be required depending on your status, such as DS-2019, I-20, etc. 

Please note that any documentation not listed above cannot be accepted as Proof of Identity.

If your name is different from the document which is presented from above (due to adoption, marriage, divorce, court-ordered name change), or is not the same on all your documents, you must provide additional documents with legal proof of every name change. If immigration documents are presented, the customer must provide proof of the updated name change with USCIS or SAVE.

Each document must be an original or a certified copy. Notarized documents are not accepted. Please provide one (1) document from the list below for every name change from your Section 1 document to your current name:

 

  • Valid, unexpired U.S. Passport
  • An official government-issued marriage certificate/license
  • Consular Report of Birth Abroad
  • Valid, unexpired Permanent Resident Card
  • Naturalization/Citizenship Certificate
  • Valid, unexpired Employment Authorization Card
  • Valid, unexpired foreign passport with a valid U.S. visa and valid I-94 issued by DHS. Other USCIS documents may be required depending on your status (DS-2019, I-20, etc.).
  • An official divorce decree
  • A court-ordered name change
  • Valid, unexpired REAL ID from another state 
  • Valid, unexpired U.S. Department of Defense ID, including IDs issued to dependents (military ID)

Social Security Number

For some services, you will be asked for your Social Security number. Your physical social security card is not required - it is only required that you know your number.


Proof of Oklahoma Residency Documents

The following documents must include your name and the residential address:

Must be dated within the last 60 days:

Monthly statements, examples include:

  • Utility bills
  • Electricity bills
  • Telephone Bills, including cell phone
  • Water bills
  • Sewer bills
  • Cable bills
  • Satellite cable bills
  • Heating oil
  • Propane provider
  • Homeowner’s or renter’s insurance 
  • Bank statement 
  • Credit card statement    

Correspondence delivered by the U.S. Postal Service, FedEx, or UPS, not including junk mail or handwritten letters

Must be dated within the last year:

  • Copy of previous year tax return filing, either federal or Oklahoma 
  • W-2 wage or 1099 tax form
  • Property tax bill or receipt
  • Social Security Administration (SSA) document 
  • Correspondence from a federal or Oklahoma governmental agency
  • Public assistance benefit card and correspondence, including correspondence from homeless shelter, halfway house or transitional service provider verifying that the provider receives mail for the customer

Active and valid

  • Valid, unexpired Oklahoma driver license or identification card*
  • Oklahoma Motor Vehicle Title or Registration
  • Oklahoma Boat Title or Registration
  • Oklahoma Voter Registration Card
  • Residential lease 
  • Mortgage
  • Deed
  • Insurance policy or card with address including homeowner’s, renter’s, life, health, or vehicle insurance
  • Installment loan contract from a bank or other financial institution, not to include documents reflecting a cash advance or signature loan
  • Professional license issued by an Oklahoma governmental agency
  • Government issued pilot license
  • Valid handgun permit
  • Documents issued by the US Military
  • Transportation Security Administration (TSA) letter
  • Tribal vehicle titles or registration
  • Certified copy of court order
  • Oklahoma high school, college, university, or technology center transcript
  • Sales tax or business license
  • Oklahoma Agriculture Exemption Permit/Farm Tax Permit
  • Address Confidentiality Program
  • Affirmation of Shared Residency* (for REAL ID only)

* Not for CDL purposes 


Proof of Lawful Presence Documents

Each document must be an original or a certified copy. Notarized documents are not accepted.

  • Birth certificate
  • Consular Report of Birth Abroad issued by the U.S. Department of State (Form FS-240, DS-1350, or FS-545)
  • Valid, unexpired U.S. Passport or Passport card
  • Valid, unexpired Permanent Resident Card issued by U.S. Department of Homeland Security or USCIS (Form I-551)
