Birth Registration

A birth certifier is responsible for accurately completing and submitting birth certificate information within required timeframes, in accordance with Oklahoma law. The birth clerk will work with the parent(s) to gather the information needed to register the record. Once gathered, they will provide a copy to the parents to review and approve prior to completing the registration and submitting the official state record to Vital Records for permanent filing. 

Having a child can be an exciting time, and there is much to do to begin the journey of parenthood. It will be important to start out on the right foot. Taking the time to review the information gathered by the birth clerk will help ensure an accurate registration. Providing accurate information at the time of birth, can save both time and money. Once a record is registered by the hospital, an amendment request would be required to change or correct any information listed on the record.

What information should I review to ensure accuracy on my child’s birth record?

• Correct spelling of first, middle and last name(s) of parent(s) and child
• Dates of birth listed for parent(s) and child
• Parent’s social security numbers
• Place of birth for parent(s)
  • If a parent does not have a social security number, please communicate that to the facility of birth

Filing: the presentation of a certificate of birth, death, or stillbirth for registration by the State Commissioner of Health. (63 OS 1-301.3)

Registration: the acceptance by the State Commissioner of Health and the incorporation in his official records of certificates of births, deaths, and stillbirths. (63 OS 1-301.4)

Live birth: the complete expulsion or extraction from the mother of a product of human conception, irrespective of the duration of pregnancy, which, after such expulsion or extraction, breathes or shows any other evidence of life such as beating of the heart, pulsation of the umbilical cord or definite movement of voluntary muscles, whether or not the umbilical cord has been cut or the placenta is attached. Certificate of birth resulting in stillbirth: a certificate issued to memorialize a stillborn child. (63 OS 1-301.7)

Fetal death (stillbirth): death prior to the complete expulsion or extraction from its mother of a product of human conception after a period of gestation as prescribed by the State Board of Health. The death is indicated by the fact that, after such expulsion or extraction, the fetus does not breathe or show any other evidence of life such as beating of the heart, pulsation of the umbilical cord or definite movement of voluntary muscles. Physician: a person who is a member of the class of persons authorized to use the term “physician” pursuant to Section 725.2 of Title 59 of the Oklahoma Statutes. (63 OS 1-301.11)

Institution: any establishment, public or private, which provides inpatient medical, surgical or diagnostic care or treatment, or nursing, custodial or domiciliary care, to two or more unrelated individuals, or to which persons are committed by law. (63 OS 1-301.12)
