Home Services Division
The purpose of the Home Services Division is to ensure and monitor compliance of minimum criteria for the issuance, maintenance and renewal of home care, companion sitter service, and hospice agency licenses. HSD staff perform on-site surveys, investigate complaints, conduct home visits to clients receiving services, and sanction agencies that fail to comply.
- Oklahoma licensure of home care agencies is required by Oklahoma Statute Title 63, Section 1-1960 et seq.

- Oklahoma licensure of hospice agencies is required by Oklahoma Statute Title 63, Section 1-860, 1 et seq.

- Medicare certification requirements may be found at Centers for Medicare & Medicaid Services
All licensing fees must accompany the completed application form and be mailed to the following address: Financial Management-Receipting Unit, OSDH, PO Box 268823, Oklahoma City, OK 73126-8813. Failure to do so may result in a delay in the processing of your license application.
