HealthChoice is excited to offer five CARE programs for eligible plan participants. Our CARE programs offer expert clinical support and tools for reaching your personal health goals. Participation in all programs is voluntary, and all resources are available at no cost to participants through the HealthChoice benefit plans.
Ongoing Condition CARE Program
Personalized guidance from a CARE nurse for individuals living with diabetes, high blood pressure, heart disease, asthma or other serious health conditions. Because these conditions can affect a person's mental and emotional health, CARE professionals also help participants manage symptoms of depression.
Maternity CARE Program
We know you’ve likely already chosen a trusted doctor or other skilled healthcare provider to manage your care and wellness needs. This program offers one-on-one guidance and educational resources from an OB/GYN nurse as supplemental support to answer quick questions about your body’s changes and your baby’s growth.
* A $25 reward card is offered to participants who sign up during their first or second trimester and complete the program.
Wellness CARE Program
Designed to help members make healthier choices and build healthier habits by generating excitement and motivation through social connections. The HealthChoice Wellness Coordinator will facilitate fun, interactive opportunities for members to learn more about fitness, nutrition and lifelong habits like sleep and stress management.

Complex Condition CARE Program
A dedicated CARE nurse will work directly with your providers, directly with your doctor and/or other health care representatives to coordinate your care and help you navigate the health care system, feel confident about your health care decisions and understand your treatment plan and doctor’s instructions.
CARE Ques
A new initiative to provide personalized reminders of actions individuals can take to improve personal health. Reminders include prompts to recommend preventive care, such as annual screenings, mammograms or cervical cancer screenings, diabetes monitoring or heart health care.
