Location Change Requests

To request a location change for your business license, follow the steps below:

1. Log in to the Thentia licensing portal.
2. Select business portal.
3. Select the business profile.
4. Select the license for which you would like to request a location change.
5. Select location change request. 
6. Fill out the required fields and upload all required documentation. The application must be fully completed, meeting all requirements.
7. Once you complete the application and submit payment, the application will be sent to OMMA for review.
