Check Box and Fill Out RESERVOIR DEWATERING PROJECT section and other sections that are applicable

Minimum required attachments:

• A) Initial production test or data indicating 5:1 water/oil or water/gas ratio
• B) Structure and isopach maps of applicable reservoir
• C) Schematic diagram of electrical grid and dewatering and water disposal equipment

Check Box and Fill Out ENHANCED RECOVERY PROJECT section and other sections that are applicable

Minimum required attachments:

• A production test or data showing oil production indicates total content of oil recovery after the use of enhanced recovery methods did not exceed 1% by volume
• Structure and isopach maps of applicable reservoir
• Map and schematic diagram of producing, injection and disposal wells and other water injection or circulating equipment.
