Birth Certificate Eligibility

Authorized Birth Certificate Applicants and Required IDs

Applicant is the:
Subject of the Record (must be age 15 or older)
  • Complete Application
  • Signature of the Subject on the Application
  • Fees
  • 1 primary form of ID of the Subject OR 2 forms of secondary ID of the Subject (If using this option, the certificate MUST be mailed to the applicant at the address on the IDs.)
Parent named on record
  • Complete Application
  • Signature of the Applicant on the Application
  • Fees
  • 1 primary form of ID of the Parent OR 2 forms of secondary ID of the Parent (If using secondary ID, the certificate MUST be mailed to the parent at the address on the IDs.)
Attorney for Subject or Adoptive Parents Complete Application
Adoptive Parents   Signature of the Applicant on the Application
Rep with Power of Attorney document Fees  
Legal Guardian 1 primary form of ID of the Applicant  
Court Order
  • Proof of relationship to the subject (e.g. client agreement, court order, notarized statement from parent, etc.)
  • If attorney is using a runner
    • Letter from attorney on firm letterhead identifying the runner
Foster Parent Contact case worker to obtain a copy of birth record

Genealogist: Open Records

  • Births occurring 125+ years ago
  • Complete Application
  • Signature of the Applicant on the Application
  • Fees
  • 1 primary form of ID of the Applicant

Genealogist: Closed Records

  • Births occurring less than 125 years ago
  • Complete Application
  • Signature of the Applicant on the Application
  • Fees
  • 1 primary form of ID of the Applicant
  • If the subject is alive: a statement signed by the Subject releasing record to the Applicant and a copy of the Subjects ID
  • If the subject is deceased:
    • a statement signed by a family member, proof of familial relationship to the decedent, and a copy of the family member’s ID and proof of death (if the death occurred outside Oklahoma)

1) a statement signed by a family member, proof of familial relationship to the decedent, and a copy of the family member’s ID and proof of death (if the death occurred outside Oklahoma)

OR

2) court order

  • Other: including but not limited to a spouse, stepparent, grandparent, relative, friend
  • Complete Application
  • Signature of the Applicant on the Application
  • Fees
  • Statement signed by the Subject (or a named parent if the subject is under age 18) releasing the record to the Applicant
  • 1 primary form of ID of the Subject (or named parent if the subject is under age 18)                                     
  • 1 primary form of ID of the Applicant

Attestation (Subject is applying for his/her own record, 2nd party is attesting to Subject’s identify)

!! NO 3rd Party Vouching (i.e. a friend cannot vouch for a parent to obtain a child’s certificate) Check allowable applicant list.

    • If subject < 18, school statement including name, date of birth, and current address
  • 1 primary form of ID of the Attester/2nd party
