You want to be a ...

Licensed Midwife

You have expressed interest in becoming a Licensed Midwife. To be eligible for a Midwife License, you must:

  • Be at least 18 years old.
  • Be a high school graduate or possess a GED.
  • Submit an affidavit of lawful presence.
  • Submit a correctly completed license application and $1,000.00 fee through the licensing portal.
  • Have a successful background check.
  • Provide a copy of current CPR certification.
  • Provide a copy of current certification in neonatal resuscitation by the American Academy of Pediatrics or equivalent.
  • Provide a copy of current certification from the North American Registry of Midwives, American Midwife Certification Board, or other Commissioner-approved organization.
  • Provide a copy of current Blood Borne Pathogen certification.
  • Provide proof of completion or a training certificate, dated within the last three years in administration of medication, including injections and IV administration.
  • Provide a resume with relevant work history.
  • Provide a list of other credentials.
  • Disclose any prior misconduct or disciplinary actions.
  • Disclose previous court judgements related to midwifery care.
  • Have your application and supporting documentation reviewed by the Advisory Committee of Midwifery.
